Passports are important travel documents issued by the United States that should be safeguarded at all times. Should you experience a stolen U S online passport, you have to take immediate action. The State Department must be notified of your loss immediately and completion of the DS-64 form should be posted.
Accuracy
Always proofread all items that have been filled out and correct any misspelled information as this will prevent delays and denial in processing the application. Often, a police report also has to be completed and submitted together with application for a replacement booklet when submitting the necessary paperwork. This report is necessary for proof of the theft and details the events surrounding its disappearance.
Loss of a Valid Passport

Identification
One will need to present a certified birth certificate issued in the United States or Certificate of Naturalization. An identification driver license or government issued identification card is necessary as well as 2 photographs that are passport size. A fee is charged when applying for a new legal document and additional charges apply to expedited damaged American passport.
